I have been offered and declined the purchase of travel insurance which covers trip cancellation, interruption and medical emergencies. I understand that The Luxury Embark, LLC has strongly recommend purchasing travel insurance to protect my trip investment.

I understand by declining to purchase travel insurance, if I cancel or interrupt my trip for any reason, portions of this package may not be refundable and supplier cancellation penalties will apply resulting in the loss of monies up to the full cost of my trip investment.

If you decide to take on our recommendation to purchase a Travel / Medical Insurance, kindly do contact us to plan this accordingly, ideally within 10 days of your first down payment, and not later than 15 days prior departure time.

I am refusing travel insurance for my trip.
The risks for declining coverage have been explained to me by my Travel Advisor.
In not purchasing a travel insurance, I understand the risks involved in declining coverage. Declining travel insurance means I will lose all benefits of coverage and therefore fully and willingly accept that risk and that I am aware that my own medical insurance may not cover me outside of the United States.
I UNDERSTAND THAT I WILL NOT BE COVERED FOR:
- Lost luggage
- Missed connections or train delays
- Emergency air lift assistance
- Emergency medical and/or dental assistance
- Emergencies at home affecting me and/or my immediate family (spouse / partner, children, grandchildren, aunts, uncles, brothers, sisters, nephews, nieces)
- Death of myself and/or my immediate family (spouse / partner, children, grandchildren, aunts, uncles, brothers, sisters, nephews, nieces)
- Supplier default and/or bankruptcy
- Any other covered circumstances as outlined in the offered travel insurance policy
- I understand that I may lose up to 100% of the cost of my cruise/vacation package/travel arrangements, plus any cancellation fees from The Luxury Embark, LLC and/or other suppliers, if my trip is cancelled or delayed.

I hereby release The Luxury Embark, LLC, its administration, personnel, and my Travel Advisor from responsibility for any consequences, both known and unknown, resulting from my refusal of trip cancellation insurance.

I understand that The Luxury Embark, LLC has advised me of the importance of such insurance and I will not hold them responsible for any risks or lost funds. I confirm that I do not expect The Luxury Embark, LLC to assist me in any way if my trip is cancelled or delayed for any covered reason.
